Eden Brown Synergy is recruiting for an experienced Level 3 Qualified Social Worker to join the Preparing for Adulthood / Transitions Team within Leicester City Council on a 6-month maternity cover contract.
Location: Leicester City
Pay Rate: £34 per hour
Contract: 6 Months (Maternity Cover)
Working Pattern: Hybrid (Office day every Thursday)
About the Role
You will be working as part of a busy and supportive team that focuses on helping young people with additional needs transition smoothly into adulthood. The role involves working closely with families, health professionals, and education partners to ensure the right support is in place for each individual.
Key Responsibilities
* Undertake Court of Protection (COP) work, Safeguarding, Care Act, and Mental Capacity (MC) assessments
* Work in partnership with service users, families, and multi‑agency professionals
* Support individuals transitioning from hospital and educational placements into supported living
* Collaborate with children's services to ensure effective transitions into adulthood
* Promote independence and support individuals with commissioned services and direct payments
* Participate in team huddles, demonstrate proactive practice, and manage caseloads effectively
* Work flexibly and creatively to meet diverse needs
* Use Liquid Logic confidently for case management
* Demonstrate a solid understanding of the Care Act 2014 and Mental Capacity Act (MCA)
Requirements
* Qualified Social Worker with registration with Social Work England
* Experience in adult social care or transitions work
* Strong working knowledge of Care Act and MCA
* Good IT skills, including familiarity with Liquid Logic
* Ability to work both independently and as part of a multidisciplinary team
