Contract role, Living Well Team
We are recruiting for experienced Adult’s Social Worker in the Reading area.
1. We need an experienced Social Worker to work with our Living Well Team. The Social Worker will work closely with an Occupational Therapist and Commissioning colleagues to support service users accessing Supported Living services, with the aim of maximising their independence and ensuring support is tailored to their needs.
Key responsibilities:

You will undertake assessments and reviews for adults in supported living, working with providers to ensure care packages are fit for purpose, hours commissioned are appropriate to the service users’ individual needs and outcomes, and resources represent value for money.
The role will involve optimising funding resources through effective case management, developing personalised support plans that promote independence and wellbeing, and working collaboratively with multi-disciplinary teams, service providers, and service users to achieve positive outcomes.
Essential experience:
3. Previous experience working within a supported living context.
4. Demonstrated ability to deliver cost savings through effective, outcome-focused practice.
5. Strong assessment, review, and care-planning skills.
6. Ability to work effectively in partnership with multi-disciplinary teams, providers, and service users.
7. Additional information:
8. This role will require regular on-site presence at the Civic Offices, Reading for meetings and case-related site visits within Reading and surrounding areas – typically 2-3 days per week.
9. The remainder of the week can be worked remotely, subject to service requirements.
