Overview
An exciting opportunity has arisen for a Band 6 Nurse Referral Advisor to be part of a multidisciplinary team based with the Single Point of Access (SPA) as part of our Trust’s commitment to deliver excellent and responsive assessment. The successful candidate will have the opportunity to develop within our evolving PCCT team and shape the vision of an inclusive Care Navigation Centre. We have been the first in the Midlands to develop a true Single Point of Access, and we would continue to be first in many other endeavours with your help.
Role and Team
You will be working within a friendly team and be part of expanding a new and exciting service, incorporating streaming within the Emergency Department. Appropriate training will be given.
Responsibilities
* Provide management and support for all patients to access appropriate care as dictated by their clinical requirements, including access to same day emergency care, admission or assessment, and identifying other community and alternative pathways.
* Support referrals from a range of sources including GPs, clinicians, WMAS and NHS 111, and work to improve the patient’s hospital journey and reduce Emergency Department waiting times by reducing unnecessary attendances.
* Direct patients to the most appropriate care facility using a clinical triage tool, leveraging enhanced nursing assessment and ability to communicate across a wide range of specialties and clinical groups.
Qualifications and Experience
* Experience in Enhanced Nursing Assessment.
* Experience in Emergency Department (ED) is a requirement for this role.
