What you will do: - Conduct comprehensive functional assessments to evaluate patients' physical capabilities and needs within social care environments. - Plan and coordinate discharge arrangements to ensure smooth transitions from hospital to community or social care settings. - Apply physiotherapy expertise to support social care interventions, enhancing patient independence and quality of life. - Collaborate with multidisciplinary teams including social workers, nurses, and other healthcare professionals to deliver integrated care. - Maintain accurate and timely documentation of assessments, treatment plans, and patient progress. - Contribute to service development and quality improvement initiatives within the Functional Assessment Team. Qualifications and Registration: - Degree in Physiotherapy or equivalent qualification. - Registered with the Health and Care Professions Council (HCPC). Experience: - Minimum of 2 years' post-qualification experience in physiotherapy. - Experience in social care settings or functional assessment is advantageous. Skills and Attributes: - Strong clinical assessment and decision-making skills. - Effective communication and interpersonal skills to liaise with patients, families, and multidisciplinary teams. - Ability to work independently and manage a caseload efficiently. - Preferable to hold a current driving licence for travel between sites.
