Long Term Conditions Nurse - Highbridge / Burnham / Berrow (Somerset)
Overview
We are seeking an experienced practitioner to provide care within general practice settings, focusing on long-term conditions. The role may involve advanced clinical decision-making, collaborative care planning, and liaison with the broader primary care team and community stakeholders.
Responsibilities
* Act as an Advanced Nurse Practitioner, conducting initial history taking, clinical assessment, diagnosis, treatment, and evaluation of care for presenting patients within the general practice.
* Demonstrate safe, evidence‑based clinical decision making and expert care with advanced assessment and diagnostic skills.
* Meet National MNC requirements for advanced practice as applicable.
* Act as an Enhanced Care Practitioner (paramedic), providing care to presenting patients and developing the role with experience.
* Work closely with GPs to advise on treatment and healthcare for patients in primary care and community settings.
* Attend home visits (including residential and nursing homes) and liaise with staff, families and other stakeholders to develop care plans and share information with other providers.
* Triaging acute patients in the surgery environment with safe, clinical decision making and expert care.
* Collaborate with the multidisciplinary team to meet patient needs and support policy and procedural delivery.
Qualifications and Requirements
* Current registration with the appropriate nursing council (e.g., NMC) and eligibility for advanced practice in primary care.
* Experience as an Advanced Nurse Practitioner or Enhanced Care Practitioner, with demonstrated clinical decision‑making skills.
* Ability to work collaboratively with GPs and other healthcare professionals in a primary care setting.
* Willingness to develop the role over time as experience is gained.
