Job Summary
Central and North West London NHS Foundation Trust is seeking a Band 5 Primary Care Staff Nurse to join its Health & Justice Team at HMP/YOI Bronzefield. The role involves providing primary care, mental health, intellectual disability and substance misuse services to patients in secure environments. The position is a 12‑month preceptorship programme with opportunities for ongoing development and a pathway to Band 5 and Band 5 Development programmes.
Key Responsibilities
* Provide an accessible individual patient assessment, treatment and referral service.
* Monitor ongoing health status of patients and address identified issues.
* Work with patients to influence health outcomes arising from lifestyle choices.
* Implement effective discharge plans for transfer or release.
* Co‑ordinate multidisciplinary clinical practice to deliver focused integrated care and treatment programmes.
* Collaborate with staff of other disciplines, including officers and governors, to provide integrated and coordinated treatment plans.
* Triage patient needs in accordance with standard operating procedures and national policy such as NICE guidance.
* Assess, plan and implement care for patients within each care/treatment pathway to promote well‑being.
* Facilitate the use of evidence‑based practice to ensure effective and appropriate clinical care.
* Carry out general nursing duties and maintain contemporaneous records.
* Act as a preceptor/mentor where appropriate.
Qualifications and Experience
* Registered Nurse (Adult) with proficiency in numeracy skills required for safe medication administration.
* Previous experience of working in primary care services, either as a Band 5 Nurse or a Student Nurse.
* Evidence of relevant continuous professional development is desirable.

* Understanding of the Mental Health Act and its impact on patient care.
* Knowledge of infection control and medicines management.
* Ability to mentor, teach, support and supervise others within the team.

* Desirable: wound care, ear care, venepuncture, chronic disease management, smoking cessation, cervical screening.

* Annual salary: £33,677 to £40,996 per annum inclusive of Fringe HCAS (pro‑rated for part‑time).
