Relief Bank - Personal Health and Wellbeing Practitioner

ABOUT THE ROLE

As a Personal Health and Wellbeing Practitioner, you will provide a person-centred practical and emotional support service with therapeutic interventions that is responsive and caring. You will create an enabling environment and service for the given residents/participants in line with their agreed care plans and risk assessments, offering practical care that promotes the rights of the individual, preserving dignity, choice, privacy, independence, fulfilment, and personal goals.

Shift Pattern: Rota will include days, nights, evenings, weekends, and bank holiday working which can vary between 07:00 - 20:30, 19:30 - 07:30. Please note we have a priority for workers who would prefer working nights, although other shifts may also be available.

ABOUT THE SERVICE

Aspinden Care Home (ACH) is a CQC registered care home based in Bermondsey, South East London. It has a national remit and cares for those with complex needs and/or those with long-term alcohol addiction. ACH offers residents the chance to gain stability in their lives and improve their physical health and mental well-being.

ABOUT YOU

We’re looking for someone with a genuine passion and felt purpose to help people, challenge stigma, and make a real difference to people’s lives! We are looking for a team player, driven to provide high-quality care and support to others, and an effective communicator who can build rapport with others from various backgrounds. You will be compassionate, supportive, and empowering to others while able to form effective, positive, and motivational relationships.

* Act as the key worker for a caseload of residents and participants and contribute to the development of care plans, risk assessments, and reviews.
* Participate in, plan, and deliver meaningful activities and encourage residents and participants to participate in the running and development of the activities to promote their health, wellbeing, and positive engagement levels, including life skills training, group work, and psychological and alcohol-based interventions.
* Encourage social initiatives, training, and volunteering opportunities in service.
* Recognise signs of deteriorating mental and physical health and initiate appropriate interventions to prevent a crisis.
* Support named residents/participants with various personal needs as required.
* Support with and monitor residents’ healthcare and nutrition throughout their residence.
* Where appropriate, enable residents by assisting them with daily tasks such as preparing meals, doing laundry, undertaking household tasks, personal hygiene, grooming, and medication.
* Develop and maintain strong relationships with internal and external stakeholders.
* Attend appointments with residents as required.
* Admin will vary.

Other responsibilities than those described above may be required from time to time and will be expected to be performed as long as it is within the capability and level of the position.

KEY CRITERIA

What we are looking for:

* Experience of working with people of complex backgrounds, ex-offenders, mental health, substance misuse, challenging behaviours or a good understanding of the sector.
* IT proficiency, ability to learn new software programs, basic Microsoft experience.
* Understanding of the housing and social needs of people with multiple and complex needs.
* Understanding and/or practical application of key legislation regarding social care, housing, criminal justice, and mental health.
* Understanding and/or practical knowledge of the social and societal marginalisation that can be attached to people with mental health issues, addiction, exploitation, homelessness, and within the criminal justice system.

What we would like, but not essential:

* Experience creating co-produced support plans and providing appropriate interventions for service users and liaising with other professionals.
* Appropriate professional qualification: NVQ/Diploma Level 2 in Health & Social Care/Community Justice or professional equivalent.
* Experience of providing housing support and practical assistance within a residential or outreach support role.
