Advanced Nurse Practitioner – Primary Care
The Role
Primary care is where advanced clinical skills make the greatest difference. It is where you assess, diagnose and treat patients at the point of first contact. It is where clinical expertise, autonomy and decision‑making come together to deliver safe, effective care for people who may otherwise struggle to access health‑care services. At IC24 we are looking for an Advanced Nurse Practitioner who will combine clinical leadership, autonomous practice and the opportunity to improve outcomes for some of the most vulnerable patients in our community. Based within our Hastings Primary Care Hub, you will lead on delivering high‑quality care to unregistered patients, working as part of a multidisciplinary team to assess, diagnose, treat and navigate patients to the most appropriate care pathway.
The role also involves working closely with local partners, including the Seaview Project, supporting individuals experiencing homelessness, those living in temporary accommodation and patients with complex health and socialcare needs. This includes people affected by mental ill health, substance misuse, learning disabilities and other barriers to accessing healthcare.
What you’ll focus on
- Delivering advanced clinical care – Provide assessment, diagnosis and treatment at the point of first contact, managing patients with minor illness and undifferentiated presentations while delivering safe, evidence‑based care.
- Prescribing and clinical decision‑making – As an Independent Prescriber, prescribe medications following clinical examination and assessment, ensuring treatment plans are appropriate, effective and aligned to current best practice.
- Supporting vulnerable populations – Work with unregistered patients and those facing significant health inequalities, helping improve access to healthcare and supporting patients with complex physical, psychological and social needs.
- Navigating patients to the right care – Determine the most appropriate care pathway for each patient, whether that is treatment and discharge, onward referral or navigation into alternative healthcare services.
- Driving quality and clinical governance – Work within IC24’s clinical governance framework, contributing to audits, quality improvement initiatives, service development and the continuous enhancement of patient care.
- Working collaboratively – Build strong working relationships across the organisation and with external partners, contributing to shared learning, locality meetings and collaborative approaches to patient care.
- Developing services and supporting innovation – Contribute to the ongoing development of the Hastings Primary Care Hub, helping shape services, share best practice and improve outcomes for patients and local communities.
What matters in this role
- First Level Registered Nurse with current NMC registration.
- Independent Prescriber qualification.
- Completion of a Physical Assessment of Adults module and a recognised Minor Illness qualification.
- At least two years’ experience working within General Practice or Urgent Care.
- Excellent clinical assessment and decision‑making skills.
- Confidence working autonomously and managing clinical risk.
- Experience of service improvement, audit and clinical governance.
- Effective communication with patients, colleagues and partner organisations.
- Commitment to continuous professional development and evidence‑based practice.
- Ability to thrive in a dynamic environment and enjoy working collaboratively across services.

Due to the nature of this position, employment is subject to proof of eligibility to work in the UK, completion of a satisfactory enhanced DBS with barred list disclosure and three years of references.
