Assertive Outreach Senior Outreach Worker / Care Coordinator
An exciting opportunity has presented for a qualified nurse or social worker to work with service users within the Assertive Outreach Team.
Job Summary:
To act as care coordinator and lead professional within the area for service users in NE Lincs. This involves assessment, developing, implementing, and evaluating care plans using a person-centred approach.
Our ideal candidate would be a motivated, committed team player, able to work independently with initiative, and passionate about promoting good quality care to maximize independence.
Main duties of the job
The role is open to Band 5, depending on experience, and Band 6 applicants, and involves managing a caseload of patients with complex needs, working directly with patients, their families, and supporting services to provide holistic support and care within a community setting. You will be a named key worker for your caseload, but we utilize a team approach, and applicants must demonstrate strong teamwork skills to meet the demands of our service users and the service itself.
Applicants must have a flexible and innovative approach to promote engagement with the service.

Job responsibilities
Main Duties / Responsibilities:
1. Communication & relationship skills: Work with all staff within PCNs and Navigo, liaise with health and social care professionals, participate in multidisciplinary meetings, and communicate complex or sensitive information effectively. Maintain accurate records and participate in supervision and training.
2. Knowledge, training, and experience: Attend relevant training, maintain evidence-based practice, participate in team and clinical meetings, and support training of new staff and students. Have specialist knowledge related to older people and mental health, and provide advice to other agencies.
3. Assessment and decision-making: Conduct comprehensive assessments, make treatment and care plan decisions, and work under supervision to prioritize and coordinate workload.
4. Planning & organizational skills: Prioritize and manage workload independently, respond to crises, and participate in caseload management.
5. Physical skills: Be trained in control and restraint techniques, undertake regular updates.
6. Patient/service user care: Review risks, provide advice, initiate support, participate in governance, and contribute to service development.
7. Resources and human resources: Use resources responsibly, support supervision strategies, support induction and training of staff and students.

Salary: £31,049 to £46,580 per year, Band 5/6 progression depending on experience.
