We are looking to find a perfectfit for our current team - a full time, experienced General Practice Nurse, whowants to work in a high achieving Practice. We expect the candidate to be ableto demonstrate a high standard of clinical excellence with experience andqualifications in long term condition management.
The ideal candidate will also beforward thinking, a great team player, have excellent communication skills, andthe ability to work collaboratively with the nursing and HCA team at thepractice, led by the Nurse Manager.
This position is funded throughARRS via Ribblesdale Primary Care Network which Sabden and Whalley MedicalGroup is a member of.
Main duties of the job
This is a full-time role working 36hours per week over 4 days, Monday to Friday, with a day off during the week. Pleasesee the attached job description and person specification for a comprehensivedescription of duties and requirements.

We are a friendly, supportiveteam situated in the beautiful Ribble Valley, with a main site in Whalley and abranch site in Sabden.
5 Partners, 6 Salaried GPs, ANP, 4Nurses, NA, 2 HCAs, and a large reception and admin team, we are also along-standing training practice for GP training. We also have HouseboundNursing Team of nursing professionals who work with our care home andhousebound patients.
We have a PCN team that work withour patients, including a Pharmacist, Pharmacy Techs, GP Assistant, CareCo-ordinator and a physio. We also have an onsite dispensary at both sites - togive all of our patients a good quality experience at the surgery.

To be responsible for the implementation of processes for theeffective management of patients with long-term conditions using evidence-basedpractice including care for elderly and housebound patients.
Working as a senior member of the practice multidisciplinary team,the post holder will ensure nursing services are delivered effectively to theentitled patient population.
The Practice Nurse will be responsible for a number of clinicalareas such as infectionprevention and control, health promotion, chronicdisease management, health promotion, well women and well man clinics, as wellas actively supporting the practice management team in the reviewing anddelivery of clinical policy and procedure.
This position is funded through ARRS viaRibblesdale Primary Care Network which Sabden and Whalley Medical Group is amember of.

Experience
* - Experience of working in a primary care environment
