An exciting opportunity has arisen with the introduction of a new Discharge Team at Salisbury NHS Foundation Trust. We are seeking an experienced, self‑motivated, and passionate individual to join our Hospital Discharge team within the acute setting.

The post holder will be embedded within the Hospital Flow structure and will play a pivotal role in efficiently managing and overseeing discharges across inpatient wards. Working closely with the multidisciplinary team (MDT), patients and families, the Integrated Discharge Hub, and community partners, the role will support safe, timely, and coordinated discharges.

You will be an experienced healthcare professional with strong leadership skills, able to work autonomously across complex systems and build effective relationships across organisational boundaries. You will demonstrate a commitment to patient safety, equality, diversity and inclusion, and continuous service improvement.

This role involves direct contact with patients, service users, and their families, ensuring discharges are safe, appropriate, and person‑centred.


Main duties of the job

Key Responsibilities

* Provide senior clinical and operational oversight of complex discharges across multiple specialties.
* Work as part of the senior discharge leadership team, supporting decision‑making, escalation, and resolution of discharge barriers.
* Line manage and support a team of Discharge Facilitators, ensuring high standards of performance, inclusivity, and adherence to Trust values.
* Proactively identify and challenge factors that may delay discharge, working collaboratively to resolve issues.
* Support the Trust's strategic objectives for improving hospital flow.
* Achieving earlier discharges.
* Reducing overall length of stay.
* Improving system coordination across health and social care.

Experience

* Registered Nurse
* 2 years leadership in nursing (band 6)
* Evidence in a senior clinical role within last 3 years
* First degree in Nursing or subject related to Healthcare
* Leadership experience or development
* Strong communication, organisational and interpersonal skills
* Recent experience of working in the acute hospital
* Experience of multi‑professional working

£49,387 to £56,515 a year per annum pro rata if part time
