The Patient Safety Incident Investigation administrator will be responsible for working to strict timescales to provide support for the investigation of incidents that meet the requirements of the Patient Safety Incident Response Plan that occur within the Trust. The post holder will be responsible for the collation, preparation and effective case management of patient safety incident files, including requesting case notes and gathering relevant documentation evidence including national and local guidance, polices, safety alerts etc. required for use within the investigations. The post holder will support all members of the Patient Safety Incident Investigation Team in delivering an in-house investigation service which contributes to improvements within Patient Safety and the wider Quality and Safety agenda. The post holder will be responsible for planning and co-ordinating Incident Investigation and Human factors training across the Trust. The post holder will be responsible for arranging and taking minutes for the Complex Case meeting and other Trust meetings when required such as the Patient Safety Group meeting, Trust Wide Quality Governance and arranging events.
The post holder will be integral to supporting the organisations’ way of working, model its values and leadership behaviours, and champion safe, personal and effective care.
Main duties of the job
The Patient Safety Incident Administor will act on behalf of the Assistant Director of Patient Safety and Effectiveness to oversee and support the
administration of all investigations of serious patient safety incidents occurring within the Trust, ensure effective support and engagement with the Patient Safety Incidents Investigations leads.

You will be responsible for providing administration support to the Patient Safety Incident Investigation team to undertake investigative process for
a concurrent number of allocated serious patient safety incidents across the Trust, many of which will be characterised by high levels of
complex, sensitive, contentious and and at times distressing information. The post holders will be integral to organisational learning and
improving patient safety.

* Educated to A level or equivalent
Desirable criteria
* RCA 3 or equivalent Minute taking qualifications and experience
* ECDL qualification or equivalent

* Experience of working in a busy business administration environment

* Experience of using electronic filing / case management system
* Experience of complaints and serious incident frameworks within the NHS
* Able to effectively communicate highly complex information both verbally and in writing to a wide range of audiences
* Ability to identify issues that require escalation to or advice from a PSII Lead or Assistant
* Able to adopt a range of communication styles appropriate to the situation and audience
* Ability to analyse and interpret medical records, reports from clinicians
