Job overview

We are looking for experienced Band 5 Nurses with a community/acute background, to support our new, innovative Team to reduce emergency hospital admissions and to reduce the reliance on acute hospital care. We have several new initiatives relating to integrated Community-based Frailty Hubs, increased community services and the implementation of virtual wards. Virtual wards support patients, who would otherwise be in hospital, to receive acute care, remote monitoring, and treatment that they may need in their own home or usual place of residence.

The post holder will support the needs of any individual’s complex health issues and acute episodes of ill health requiring either an urgent 1–2-hour response, or a patient who is on the virtual ward for an average length of stay between 1-14 days, and agree care plans, utilising the single assessment process and escalate to senior staff where necessary.

Main duties of the job

The post holder will be responsible for monitoring patients and conducting holistic needs assessments of patients using recognised assessment tools and remote technology solutions.You will be able to fully recognise the deteriorating patient and escalate appropriately to support with the management of symptoms that have been identified.

You will collaborate with a team of professional leads, and in partnership with GP practices and local Primary Care Networks to further develop services which are of value to the commissioners and meet the local needs of the population.

Person specification

Education/qualification

* Diploma or Degree in Adult Nursing.
* Current registration and evidence of current CPD
* An understanding of relevant clinical practice/standards within identified clinical area
* Diploma/ Degree IV Cannulation Mentorship programme
* Willingness to undertake teaching and assessing programme.

Experience/Knowledge

* Understanding of caseload management responsibilities.
* Minimum 2 years experience post registration.
* Understanding of Lone / agile working.
* Experience of working in both acute and community settings

Skills/Abilities

* Fundamental Clinical Skills: Observations, Manual Handling Basic Life Support
* Frailty Assessment / Clinical Frailty score (Rockwood) News Score
* Ability to complete a full clinical assessment following a prescribed proforma
* Ability to work independently in the community.

Personal Qualities

* Demonstrates PRIDE values and behaviours and has a positive and proactive attitude towards work.
* Understands and demonstrates the BHRUT values.
* Professional attitude towards Nursing,trust-worthy and non- prejudicial
