Parkinson's Nurse month secondment opportunity
The Parkinson's Nurse is ahighly experienced, knowledgeable and educated member of the Parkinson’s Service and Lionel Cosin Day Hospital who is able to work autonomously making high level decisions to treat the health needs of patients with Parkinsons. The Parkinson’s Nurse will be expected to visit patients at their homes, nursing homes or community hospital.
Main duties of the job
To be clinically and professionally responsible for Parkinson’s patients throughout the pathway, providing and maintaining a high standard of care.
To maintain and continually develop specialist knowledge and skills pertinent to the clinical area.
To carry out home visits or review patients in nursing home and community hospital settings.
To actively liaise with the multidisciplinary team and the wider organisation to ensure a seamless pathway of care for service users.
Work autonomously, and take responsibility for the ongoing care and support of service users using a broad range of highly specialist complex advanced clinical nursing and decision making skills, which may include prescribing where appropriate
Contribute to ongoing service development which has a strong focus on audit, research and evidence based practice.
Practice a high level of communication skills and organisational ability to sustain partnership with individuals, groups, managers, stakeholders and industry to ensure the high profile of the service is maintained.
