We are seeking a dynamic and motivated Band 7 Physiotherapist (or an experienced Band 6 looking to progress) with strong assessment and treatment skills in stroke and neurological rehabilitation to join our Stroke Therapies Service.

Main duties of the job
6. To provide a high standard physiotherapy service to stroke and neurologically impaired adult inpatients.
7. To undertake advanced assessment and treatment of stroke and neurologically impaired and other in-patients including those who have highly complex and / or chronic presentations, to determine clinical diagnosis and treatment indicated and to maintain associated records, as an autonomous practitioner
8. To provide highly specialised advice to patients, family, carers, and health and social care professionals regarding the management of patients with highly complex neurological conditions.
9. Interpret and analyse clinical and non-clinical facts, to provide accurate recommendations on further needs and development of comprehensive discharge plans.
10. To take a lead role in the case management of patients with highly complex neurological disabilities and who require onward referral to specialist Neuro-rehabilitation and where appropriate, facilitation of transfer to appropriate interim facilities.
11. Communicate with the MDT and the patient / family re: the status of rehabilitation referrals and requirements for acceptance / transfer. This includes highlighting requirements to support referrals (completion of reports, nutritional management e.g. insertion of PEG, setting and implementation of MDT goals) to the MDT.

Experience
Essential criteria
15. Significant post-graduate working experience at Band 6 level (or equivalent) or above in stroke and neurological conditions
16. Experience managing others within a team
17. Evidence of significant knowledge/experience of MDT working
Desirable criteria
18. Experience of organising, writing and delivering in-service training programmes
19. Experience of service evaluation
Knowledge
Essential criteria
20. Ability to demonstrate on-going CPD and the use of evidence based practice.
21. Knowledge and experience of management of elderly patients, including those with falls, dementia and / or delirium
22. Comprehensive knowledge and experience of safeguarding policies, practices, procedures, and associated responsibilities
Desirable criteria
23. Demonstrates comprehensive knowledge of national clinical guidelines and standards, with particular expertise in stroke and neurology care pathways
Qualifications

25. Degree/Diploma in Physiotherapy
