What the Job Involves This role includes coordinating safe and timely discharges under the Discharge to Assess (D2A) model. You will work closely with NHS colleagues and community services to ensure statutory duties are met. Key responsibilities include implementing D2A pathways, conducting Care Act assessments, developing care plans, responding to safeguarding concerns, and attending MDT meetings. You will engage with families and maintain accurate records while adhering to local authority and NHS policies. The position requires site travel and demands compliance with necessary certifications and checks.
