About the Role
Join our multi-disciplinary team delivering recovery-focused care to adults with mental health needs in their homes, within the community or within our supported living accommodation services. As a reablement worker, you’ll help people build resilience and independence, working one-to-one and in groups
Job Opportunity
Main Responsibilities
* Conduct assessments, develop and execute person-centred Care/Recovery Plans
* Provide 1:1 motivational support, coach clients toward personal goals
* Facilitate skills-building groups and connect clients to community services
* Liaise with multi-agency partners (mental health services, social services, LA, housing, probation, police)
* Maintain accurate records, complete risk and incident reports in compliance with GDPR and safeguarding
* Attend clinical supervision, team meetings, and training sessions

Essential Skills

· Strong communication skills – both listening and report writing

· Able to work a range of shifts which make include evenings/weekends and sleep-ins
· Up to date understanding of safeguarding, equality and confidentiality.
Desirable:
· Experience working in a recovery setting within mental health
· Driving licence and own car
Knowledge of local service (housing/support groups/probation etc)
