Our Primary Care Liaison Team is at the heart of connecting people with the right support at the right time.
We provide initial assessments and support conversations for individuals who may require secondary mental health services. Working closely with GPs, voluntary sector organizations, and both primary and secondary care, we ensure that people receive tailored, compassionate, and effective care. For example, we may meet someone who we then signpost to available primary care support, or identify cases requiring referral to secondary mental health services.
This role is a development position with regular supervision, with the aim that you will progress to a Band 6 Specialist Practitioner role within an agreed timeframe.

Key responsibilities include:
1. Completing initial assessments and support conversations for referred individuals.
2. Managing urgent and routine referrals.
3. Formulating safe, effective, and person-centered care plans.
4. Making clinical decisions under supervision, ensuring robust risk management.
5. Liaising with primary care, secondary mental health services, and voluntary organizations.
