About the Role
Our skilled Onwards Team provides support to individuals who may have spent an extended time in hospital and needs more intensive, long-term support living in their own home. The team provides practical and psychological support to individuals with complex needs, empowering them to become as independent as possible.
Job Opportunity
The role of a reablement or support worker is community based, working with individuals in their own homes or escorting them out to activities, appointments etc; supporting engagement within their locality. Working in partnership with a multidisciplinary team, we ensure the delivery of effective support, increasing client autonomy and helping with the management of triggers and behaviours – some of which may be challenging.
This is an opportunity to work within a supportive and knowledgeable team.
This is a full time role which will include weekend work on a regular basis.
Essential Skills
Essential Skills/Experience:
* Health & Social Care Qualification OR demonstrable experience working with people with a Mental Health diagnosis.
* Able to work as part of a small team with an adaptable “can do” approach
* Able to work with our clients with a non-judgmental outlook, treating all with equality and respect
* Familiar with MS programmes & able to use Word, Excel & SharePoint
* Understanding of Data Protection rules and how these are implemented at work
* Able to work in the UK without restrictions – sponsorship is not available
Desirable Skills/Experience
* Previous experience with community-based support work
* Car owner driver for in-work travel
* Familiar with Learning disabled and neurodivergent clients & able to adapt to individual needs
* Flexible to changing rotas and priorities
* Maintain organised and accurate records/reports
* Excellent communication skills – written and verbal
* Batchelor or Master’s Degree (Psychology or similar)
