An exciting opportunity has arisen for an enthusiastic Health Visitor to work 7.5 hours per week with the Mother and Baby Unit within SBUHB.
The post holder will be responsible for delivering the health visiting programme for a defined caseload of children, families and communities to support children reach their full potential.
The Health Visitor will be expected to work in partnership with other professionals and agencies to address health needs - demonstrating excellent communication skills whilst working both autonomously and as part of a team.
Main duties of the job
This role is designed to meet the needs of the local population with the overarching aims of increasing family resilience, improving population health outcomes and reducing health inequalities.
The post holder will be responsible for the assessment of health needs and the development, implementation and evaluation of programmes of care to meet the needs of the child and their family following the Healthy Child Wales Programme.
The post holder will work with families and young children in their homes and other venues within the community to provide advice, support and interventions to help parents make decisions which positively affect their family’s future health and wellbeing.
In order to provide a seamless service for children, families and communities, the health visitor will actively work in a multi-agency environment and actively promote effective working relationships and communications with colleagues, professionals and statutory agencies.
