Responsibilities
An exciting opportunity has arisen for an experienced Assistant Practitioner (AP) to join the Locala Home Enteral Nutrition (HEN) Team in South Kirklees (Huddersfield Area) on a permanent contract. We are looking for an enthusiastic, innovative, and proactive AP with excellent clinical, communication and team working skills.
This post offers opportunities to provide clinical and admin support to the Home Enteral Nutrition Team (HEN), who manage patients in the community.
In this role, you’ll work closely with the HEN Dietitians & Nutricia Nurses to care for patients who are tube‑fed at home – many of whom have complex neurological conditions such as Motor Neurone Disease (MND), Multiple Sclerosis (MS) learning disabilities and other challenging health needs. You’ll play a key role in helping us deliver high‑quality, person‑centred care alongside other community‑based professionals such as Speech and Language Therapists, Physiotherapists, Specialist Nurses, to provide joined‑up care that supports patients in remaining at home safely. There will be opportunity to attend MDT’s and proactively contribute to support our patients.
The role requires emotional resilience and sensitivity, as you will be supporting patients and families through challenging situations, including end‑of‑life care.
What You’ll Be Doing:
* Help to order and keep track of nutritional products, feeds, supplements, prescriptions, following guidance from Dietitians. Using SystmOne and the Nutricia Homeward platform
* Registering patients on the relevant systems
* Collecting relevant patient data to support the dietitian in feed regime planning.
* Weighing patients using various types of scales (hoist, stand‑on, wheelchair) and keeping records up to date.
