We are looking for a committed, enthusiastic person to join our innovative multi-disciplinary team based in York's newest purpose-built primary care facility.
As the successful candidate, you will be a key part of our two Partner practice and will work alongside a growing team.
You will be responsible for delivering healthcare to people who live and work in York, as well as staff and students at the University of York.
Unity Health currently operates two surgeries to the South and East of York, close to excellent local amenities. We are a consistently high QOF achiever, a supportive member of the Humber & North Yorkshire ICB and an active member of the Student Health Association (SHA).

Main duties of the job
Accountable for the delivery of efficient, quality and evidencebased clinical care in accordance to professional standards, guidelines andcode of conduct (Safeguarding, Nursing and Midwifery Council, NICE, LocalAuthority Guidelines)
ChronicDisease Management including but not limitedto:
1. Asthma
2. Vascular Disease (including IHD/Stroke/PVD)
3. COPD
4. Diabetes
5. Hypertension
To undertakeclinical assessment and treatment of patients attending the Practice.
Functionin a dynamic way ready to adjust to the changing needs of the client group.
As a PracticeNurse, its the nurses responsibility to adhere to any guidelines, protocolsor best practice associated with any such additional elements of their role.
FamilyPlanning including cervical cytology screening.
Assist inthe delivery of specific campaigns ie Flu/Shingles.
Delivery of routine childhood vaccinations
Maintainstock of drugs and equipment.
Advisenurse management of equipment failure/maintenance requirements.
Contributeto QOF data collation as required.
Recordall Patient Contact on Practice Medical Record system in a timely manner.
6. ConductNew Patient Health Checks.
7. Wound Management.

Unity Health is a York-based GMS Practice, serving more than 19,000 patients. It has a long history of family medicine dating back over 60 years. It also has over 30 years experience in student health, providing healthcare to the University of York.

Qualifications / Requirements:
Registeredand ongoing membership with the Nursing and Midwifery Council (NMC) byensuring professional skills and knowledge up to date anduphold professional standards.
SafeguardingAdult & Children at Level 3 training can be provided
Certification of Enhanced Criminal Record Check with children's and/or adults barred list check(s).
