Senior Social Worker – St Mary's Hospital, Warrington
Join St Mary's Hospital in Warrington as a Senior Social Worker and be part of changing lives for the better. The role focuses on delivering excellent healthcare within a strong teamwork culture as part of a multidisciplinary team supporting patients with Acquired Brain Injuries (ABI), progressive neurological conditions, Autism Spectrum Conditions (ASC) and Deaf Mental Health problems.
Responsibilities
* Managing a complex caseload and providing social care assessments, risk assessments and pre‑admission arrangements for discharge of service users.
* Liaising with home authorities (Health, SSD and Probation) at admission and discharge, and collaborating with authorities, service users and families in decision‑making and care planning.
* Maintaining electronic/integrated records of work undertaken and completing administrative procedures.
* Contributing to the preparation of reports for Mental Health Review Tribunals, appeals to the Mental Health Act and Independent Hospital Managers, and attending hearings.
* Facilitating access to welfare and support services for patients and families.
* Providing guidance and support to junior members of the team.
Qualifications & Experience
Applicants should come from NHS or private care with experience in managing complex caseloads and be capable of leading assessments and discharge planning in a multidisciplinary environment.
