The Work & Health Coach is part of the WorkWell scheme, which is a prevention and early intervention service supporting people to overcome health conditions that affect their ability to start, stay, and succeed in work.

Location:
Based at allocated GP Surgeries across Pershore & Upton (Monday, Wednesday & Friday)
Overall Purpose:
WorkWell is a prevention and early intervention service supporting people to overcome health conditions affecting their employment capabilities. The post holder will conduct assessments to identify health barriers and provide advice to overcome them. They will manage a caseload of individuals with physical, mental, or social health barriers, providing 1:1 coaching and referring to multidisciplinary support or external services as needed.
The post holder will facilitate quick access to the service for:
* People currently working but struggling due to health issues, experiencing worsening episodes
* Employed individuals off sick who need support to return to work
* Individuals recently out of employment seeking to return in a different role
This service is for eligible adults in the Herefordshire and Worcestershire Network, with criteria including geographic location, right to work, and health-related barriers.
The service aims to support clients in progressing toward employment, maintaining employment, and reducing health inequalities through a coaching approach, holistic therapy, early intervention, and employer advice. Flexibility is essential due to environmental and resource variability.
Key Working Relationships
A. Internal: Patients, Carers, GPs, Primary Care Teams, Community Care, HR, Clinical Governance, Safeguarding.
B. External: Local Authorities, DWP / Job Centre Plus, employment support providers, employers, CVS, voluntary sector partners, and other WorkWell organizations.
Key Responsibilities:
* Receive and process referrals from various sources
* Validate participant eligibility
* Manage caseloads of individuals with health and social issues
* Conduct assessments and develop action plans
* Provide coaching and holistic support
* Refer to additional services as needed
* Work with external organizations to support clients
* Deliver group sessions when applicable
* Maintain accurate case notes and upload documentation
* Collect and report management information
* Support training and awareness activities
* Travel within the area to deliver services to clients
Other Responsibilities:
* Mentoring on a one-to-one and group basis
* Travel across the area, including rural locations
* Occasional evening and weekend work
