Oxfordshire County Council are seeking amitted and skilled Social Worker to join the Adultmunity Team in Oxfordshire. This is an excellent opportunity to make a real impact within themunity—supporting adults to live independently, safely, and with the right networks around them.
Your new role
Working within Adult Social Care, you’ll use a strength‑based approach to carry out assessments, coordinate support, and collaborate closely with health partners, families, carers, andmunity organisations. You’ll play a vital role in promoting wellbeing, resilience, and positive oues for the people you work with.Key ResponsibilitiesAs part of themunity Team, you will:
1. Support individuals under the Care Act 2014, MCA 2005, and relevant legislation.
2. Develop clear, oue‑focused support plans tailored to individual needs.
3. Work with multidisciplinary teams,munity partners, and families to achieve the best oues.
4. Provide advice on Assistive Technology, equipment, reablement services, and housing adaptations.
5. Manage a varied caseload, including duty responsibilities andmunity visits.
Maintain accurate, defensible case records and contribute to service development.
What you'll need to succeed
Essential:
6. Recognised Social Work qualification & Social Work England registration
7. Strong knowledge of Adult Social Care legislation and safeguarding
8. Ability to work both independently and within a multidisciplinary team
9. Excellentmunication, assessment, and report‑writing skills
Desirable:
10. Experience with housing adaptations, equipment, or moving/handling
11. Post‑qualifying training (BIA, AMHP, Practice Educator) or willingness to work towards it
