 
        
        Clinical Pharmacist - Community Inpatient Services
An exciting part‑time Clinical Pharmacist opportunity has arisen on the Isle of Wight. The successful applicant will work alongside senior therapists, nurses, GPs and consultants to support two inpatient community rehabilitation wards at St Mary's Hospital in Newport and provide a pharmacy service to the NHS‑funded beds at Springfield Nursing Home in Shanklin.
The post holder will receive induction training, be mentored by experienced pharmacy colleagues and play a key role in delivering a high standard clinical pharmacy and medicine management service to designated physical health inpatient services. The role will involve attending ward rounds, multidisciplinary team meetings, guiding doctors and independent prescribers and ensuring safe and appropriate use of medicines.
Main duties and responsibilities
 * Provide clinical pharmaceutical input to the physical health inpatient services and line manage the physical health inpatient services Pharmacy Technician.
 * Attend relevant ward rounds, multidisciplinary team meetings and guide doctors and independent prescribers to maximise benefit and minimise risk to patients.
 * Undertake medicines reconciliation, support Medicines Management technicians and ensure the effective supply of medicines for inpatient care.
 * Monitor patients for potential and actual adverse effects of medicines, support patient discharge planning and facilitate seamless transfer of care across all interfaces.
 * Act as a clinical role model, demonstrate professional accountability to patients and promote evidence‑based practice.
 * Contribute to quality improvement initiatives, clinical audit, teaching and training of undergraduate pharmacy students, trainee pharmacists, junior pharmacists and medicines management technicians.
 * Report and investigate medicines related incidents, monitor drug expenditure and maintain appropriate Trust policies, guidelines, PGDs and SOPs.
Key areas of practice
 * Enhance patient safety in medicines practice including medicines reconciliation, clinical professional advice, discharge planning, clinical audit and teaching.
 * Utilise pharmaceutical expertise, working in partnership with patients, carers and healthcare professionals.
 * Monitor drug usage and promote cost‑effective prescribing within the Division.
Qualifications and experience required
 * Masters Degree in Pharmacy plus pre‑registration / foundation year training or a postgraduate diploma (or MSc) in Clinical Pharmacy Practice or equivalent experience.
 * Registered with the General Pharmaceutical Council.
 * Postgraduate diploma (or MSc) in Clinical Pharmacy Practice or equivalent experience and willingness to work towards this.
 * Independent prescriber qualification.
 * Post‑registration experience working as a pharmacist and demonstration of the ability to recommend, substantiate and communicate medicine related information appropriately to patients, carers and clinical staff.
 * Experience of delivering training and education, working in a constantly changing environment, ward‑based clinical pharmacy and working with older persons.
 * Monitoring and advising on drug expenditure, undertaking clinical audit and experience of community health services or primary care services.
Additional criteria
 * Able to manage own time, work both alone and as part of a team and communicate clearly and empathetically with patients, carers and clinical staff.
 * Good oral and written communication, resilience, negotiation, flexibility, prioritisation and risk management skills.
 * Methodical approach and attention to detail, evidence‑based knowledge of prescribing, drug costs and national guidance, and the ability to influence all grades of medical staff.
