Tollerton Surgery is looking to recruit an enthusiastic and motivated practice nurse to work as part of the practice nursing team and practice multidisciplinary teams.
The successful candidate will provide appropriate care working independently in line with practice policies and protocols.
The successful candidate will be involved in identifying practice population needs, general practice nurse duties including chronic disease management and working to GMS contract, Quality and Outcomes Framework (QOF) and IIF priorities, along with national and local enhanced services.
Additionally the successful candidate will be fully supported to undertake further training required to gain qualifications and skills in any area needed for practice requirements.
20 - 25 hours per week.
Main duties of the job
The post holder will be responsible for the delivery of practice nursing services, working as part of the practice multidisciplinary team. Working autonomously, the practice nurse will be responsible for a number of clinical areas, including but not limited to: cervical smear delivery (willing to train if not qualified); health promotion; chronic disease management; management of results relating to LTC reviews and primary prevention; NHS health checks; all immunisation and vaccination programmes; contributing to junior doctor support and training; as well as supporting the management team in the reviewing of clinical policy and procedures including infection control.

Job responsibilities
a. Provide routine nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF
b. Support the Infection Prevention Control lead and to work to the IPC policy
c. Develop, implement and embed health promotion and well-being programmes
d. Develop, implement and evaluate individual treatment plans for acute and non-acute conditions
e. Development of individual treatment plans for chronic disease patients
f. Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary / specialist care as required
g. Identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects on the patients health
h. Support patients to ensure compliance with prescribed medications
i. Provide wound care (ulcer /doppler etc.) to patients
k. Request pathology services as necessary, processing, interpreting, filing and acting on results as required
l. Refer patients directly to other services and team members as appropriate
m. Maintain accurate clinical records in conjunction with extant legislation
n. Ensure read codes are used effectively
o. Maintain chronic disease registers and recall systems
q. Chaperone patients where necessary
r. Run and deliver cervical screening and all immunisations and vaccination programmes
s. Supervise and train junior staff
t. Ensure the delivery of required practices and policies such as infection control, to CQC standard

Qualifications
* Registered with NMC
* Qualified to deliver all standard practice nurse duties
* Cervical cytology (willing to train if not qualified)
* Management of diabetes diploma
* Mentor / teaching qualification
