We have an excellent opportunity for a Band 6 Mental Health Nurse to join our Community Neighbourhood Team within the Directorate of Adult Mental Health.
We are looking for individuals who can develop relationships and work in an integrated way with practitioners from outside the NHS, including Primary Care, Social Care, and the Voluntary-Community sector. Main duties of the job
The post involves working with mental health in a community setting, managing your own caseload and diary. You will work as part of a multidisciplinary team, carrying out duties autonomously and using your initiative, with a commitment to a recovery approach and collaboration with service users.
You will act as care coordinator for service users with various mental health issues, providing high-quality, recovery-focused care and promoting social inclusion. You will work in partnership with other services, including voluntary sector organizations, to support recovery.
You will participate in and chair regular Multi-Disciplinary Team (MDT) meetings, using your clinical knowledge to support colleagues. You will have opportunities to perform clinical assessments for new referrals, discussed in MDT meetings for team decisions. Our team collaborates with psychologists, occupational therapists, psychiatrists, healthcare support workers, peer support workers, and community connectors.
