About us

Healthy Corinium employs the South Cotswold Frailty Service, delivering community-based frailty care across the South Cotswolds Primary Care Network. We work in a predominantly rural area, supporting five GP practices, and are proud to be a friendly, innovative, and supportive team.

We are committed to developing a skilled and motivated workforce, enabling staff to deliver safe, high-quality, and value-for-money care that makes a real difference to our local population.

Our service provides evidence-based care for adults living with moderate to severe frailty, working closely with One Gloucestershire partners to reduce the risk of deterioration and crisis. We focus on supporting people to maintain their independence and remain in the place they call home.

We work collaboratively with system partners to promote shared decision-making, coordinated and continuous care, and to support the development of the emerging Neighbourhood model of health and care. Multidisciplinary team working is central to how we deliver care and support both patients and staff.
