Senior Social Worker – Warwickshire County Council
Warwickshire County Council is seeking an experienced Senior Social Worker to deliver high-quality social work services within hospital acute settings across North and South Warwickshire.
About the role
As a Senior Social Worker you will manage a caseload of older adults and their families and unpaid carers, undertake robust assessments and care and support planning, and work closely with a multidisciplinary team of health and social care professionals. You will travel across North and South Warwickshire, support the discharge of patients and provide specialist social work support within hospital social care settings.
Key responsibilities
* Managing a caseload of older adults, their families and unpaid carers.
* Undertaking and recording robust assessments and care and support planning.
* Communicating with customers, communities, health partners and agencies.
* Working in a fast-paced, often pressurised environment and exercising resilience.
* Supporting and developing colleagues through supervision and training.
Qualifications and experience
* Professional Social Work qualification and registration with the Health and Care Professions Council.
* Experience working with older adults in hospital or acute settings.
* Good IT skills and a dynamic, strength-based approach to working.
* Driver’s licence and ability to work 5 days over 7 days, including a weekend rota.
* Experience in adult social care and hospital social care is desirable.
