An exciting job role has recently become available in Somerset. They are looking for a new member for their Hospital Discharge Team. What will your responsibilities be? In this role, you will focus on admission avoidance and prevention, ensuring people are safe. You will use a strengths-based approach for person-centred discharge planning and collaborate with Occupational Therapists, Physiotherapists, and other medical professionals in a multi-disciplinary setting. Advocacy is key, ensuring that the voices of individuals, including carers and unpaid carers, are heard. You will encourage independence by helping people grow their confidence and make the most of available support. Additionally, you will manage complex cases, making evidence-based decisions in a fast-paced environment.
