What You Will Do
* Conduct thorough assessments and create tailored care plans in collaboration with other professionals.
* Identify social care needs and determine eligibility under the Council’s Fair Access to Care Services criteria.
* Commission, arrange, and review services to ensure they meet evolving individual needs.
* Lead and contribute to safeguarding investigations, developing and implementing protection plans.
* Advocate for service users, promoting independence, inclusion, and overall well-being.
* Work within a multidisciplinary team to enhance service quality and outcomes.
* Maintain accurate records using information systems for effective case management.
What We’re Looking For
* A recognised Social Work qualification and registration with Social Work England.
* Strong knowledge of relevant legislation, safeguarding practices, and care frameworks.
* Experience in conducting assessments and designing person-centred interventions.
