Weare looking to appoint a motivated and experienced therapy support worker tojoin the Multi-Speciality Optimisation Team (MOT) team as a Band 4 AssistantTherapy Practitioner. The successful applicant will join the integratedOccupational Therapy and Physiotherapy team, covering wards designated forthose patients who no longer have medical care needs, yet require furthertherapy intervention prior to discharge or have complex social needs.
Theteam works together to assess physical, cognitive and social needs to achieveand facilitate the best outcome for patients when leaving hospital. This willbe a new post to the team with the aim of reducing the time patients spend inhospital once they no longer require further medical intervention.
Main duties of the job
Thepost-holder should have an understanding of Physiotherapy and OccupationalTherapy and the ability to work through their respective processes. They willindependently manage a non-complex caseload within the boundaries of the jobrole & seek support as needed from registered members of the team. Thesuccessful applicant will have knowledge of discharge pathways.
Excellentcommunication skills are essential for this role. Teamwork is paramount, bothwithin the MOT and the wider-multidisciplinary team. Applicants should havewith excellent time management skills to aid individual caseloadprioritisation, alongside supporting the wider team. Experience of workingwith patients with complex physical and social needs will be beneficial inthe role.
In-servicetraining is provided and the post holder would also join the support workergroup which will provide peer support for non-registered staff. The Trust hasan excellent Organisational Learning department which provides a wide rangeof opportunities to learn & develop in a wide variety of areas.

Job responsibilities
JOBPURPOSE
Towork within the occupational therapy and physiotherapy teams providing highlyskilled and specialised clinical, administrative and organisational support,working in the Emergency and Speciality Medicine service at SJUH.
Thepost holder will work independently to manage the initial phase of thetherapy assessment pathway, will be responsible for the screening of patientsattending Elderly Medical specialist ward and determine the most appropriateinitial assessments and interventions. The post holder will manage a caseloadof patients independently and be able to work with infrequent supervisionfrom OT and Physiotherapy support. Onoccasions the post holder will assess home environments to ensure safedischarge, without the supervision of a qualified member of staff.
Dayto day supervisory support and overall management will be provided to theJoint Therapy Support Worker by occupational therapy and physiotherapy staff.
Thepost holder is responsible for:
1. Assessing and providing care andtherapy interventions for individual patients, adapting the process asindicated.
2. Assessment and intervention may takeplace in patients homes, clinics or wards.

GCSE English and Maths grade Cminimum or Functional Skills equivalent
Experience working as a therapysupport worker in an acute setting

Clinical
To accept responsibility forOccupational Therapy and Physiotherapy initial assessments to facilitaterehabilitation and discharge from hospital.
To plan and prioritise clinicaltasks.
To deliver the service usingpatient pathways and protocols to support your delivery of therapy processand taking into account each patients individual physical, social andemotional needs.
To make contact with patients toinitiate assessments and intervention.
To analyse and make decisions aboutthe most appropriate environment in which to carryout intervention.

* Relevant clinical Experience.
