Overview
Team Leader - Home Carer - Community Well-Being & Independence Team (REQAA6492) at Pembrokeshire County Council. We are recruiting a Team Leader to lead a team supporting individuals to live in their own home, involving activities of daily living such as personal care, meal preparation and medication, and to write and review care plans and complete risk assessments while ensuring standards are maintained within the service to comply with Social Care Wales and Care Inspectorate Wales.
Responsibilities
* Lead a team providing home care and community-based support to promote independence and well-being.
* Write and review care plans; complete risk assessments; monitor quality and ensure compliance with Social Care Wales and Care Inspectorate Wales standards.
* Coordinate and supervise care delivery, ensuring high-quality support and effective communication with service users and their families.
* Manage scheduling, rota and workforce planning; implement a two-week rolling rota and provide weekend enhancements as applicable.
* Provide ongoing training and development for team members; support supervision and performance management.
Qualifications
* Minimum of a level 2 in Adults Health & Social Care or equivalent; commitment to work towards level 3.
* Registration with Social Care Wales for this post is required.
* Strong written and verbal communication skills; robust leadership and organizational abilities; a positive approach to the well-being of others.
* Ability to support individuals to live well in their own home.
Contract and Salary
* Grade 6 – £28,142 - £31,022 per year
* Contractual hours: 37 per week
* Basis: Open Ended
* Weekend Working Allowance; every other weekend off
* Block pay and a two-week rolling rota; uniform, work phone, travel allowance or work van where available
