Greenock Health Centre

The post holder will provide an accessible, appropriate, high quality, culturally sensitive and effective nursing service to patients aged 16 years and over within CTAC services whilst working in partnership with other members of the primary health care team and service users. The post holder will be a first level nurse who will support the CTAC Team in delivering care to patients attending CTAC services .The post holder will be responsible for providing skilled nursing interventions for patients attending CTAC Services.

Duration, Location, and Working Pattern:

1. Greenock Health Centre are looking for a Community Treatment and Care Nurse to join their team!
2. The shift pattern for this post is 4 shifts per week over Monday to Friday within the timeframes of 08:00 to 18:00

Key responsibilities:

3. The post holder is responsible for the assessment, planning, implementation and evaluation of programmes of care. These will be developed in partnership with Patients, Family, Carers and Significant others, and will be evidence-based taking into consideration the lifestyle, race, gender and cultural background, and factors which impact on physical, mental or emotional health
4. The post holder will perform a wide range of nursing interventions and investigations for patients attending CTAC services, and will support patients, relatives and carers in order to achieve and maintain optimum health. They will also be responsible for delivery of the national vaccination programme
5. The post holder will adhere to local, regional and national policies, procedures and guidelines pertinent to their area in the pursuit of the highest standard of patient care
6. The post holder will develop leadership skills, acting as a role model by representing the values and beliefs of the nursing profession as well as that of the organisation
7. Participate in systems of clinical support and personal development plans for the nursing team, induct new staff into the team and supervise junior staff
8. The post holder is responsible for ensuring that they practice within the Legal & Ethical framework as established by Nursing & Midwifery Council (NMC) and National Legislation to ensure the patients interests and wellbeing are met
9. Maintain accurate up-to-date clinical records and care plans in accordance with NMC standards for records and record keeping, ensuring compliance within GDPR and the Freedom of Information Act
10. As a mentor (underpinned by theoretical and experiential knowledge) the post holder will ensure that systems are in place to supervise and support pre-registration nursing students. Accommodate other multi-disciplinary and multi-agency personnel who require a working knowledge of the treatment room service
11. Participate in the audit of clinical placements in partnership with Higher Education Institutions and implement any changes as required, working in collaboration with the Practice Development Nurse
12. The post holder will utilise various clinical and environmental risk assessment tools in order to identify actual and potential risks and implement appropriate interventions. The post holder will also be responsible for reporting and participating in critical analysis of incidents to reduce any recurrences
13. Maintain resuscitation equipment and have the skills to respond to emergency situations e.g. anaphylaxis and cardiac arrest, by providing basic life support

Knowledge, training, qualifications and/or experience required to do the job:

15. 1st Level Registration
16. Relevant post-registration experience & evidence of revalidation
17. Evidence of continuing professional development
18. IT skills
19. Good communication skills (both verbal and written)

21. Leadership skills
22. Ability to work as part of a team
23. The role requires travelling across the Inverclyde area to visit patients in their homes, in care home and in clinics/health centres. Vehicles are available for use if required and applicants should have a current driving licence. Reasonable adjustments will be considered for applicants where this is required
