Buckinghamshire Healthcare NHS Trust is seeking a dedicated MND Care Co-ordinator to join our team. This is a unique opportunity to provide expert care coordination across the Trust, working closely with neurologists, palliative care teams, the Oxford MND Care Centre, and the MND Association.

We’re looking for a registered healthcare professional (e.g. Nurse, AHP) with:

1. Significant experience in neurological or palliative care.
2. Advanced communication and leadership skills.
3. A commitment to holistic, patient-centred care.
4. The ability to work across settings including homes, hospitals, and care homes.
5. A proactive, innovative approach to service development.

Main duties of the job

9. Lead and coordinate care for people living with MND from diagnosis to end of life.
10. Act as a specialist resource for patients, families, and professionals.
11. Develop and implement care pathways and service improvements.
12. Provide education and support to staff and carers.
13. Advocate for patients and promote clinical excellence in line with NICE guidelines and MND Association standards.

Person specification

EDUCATION, QUALIFICATIONS & TRAINING

Essential criteria

19. Registered healthcare professional (e.g. Nurse, Allied Health Professional) with current professional registration
20. Evidence of continued professional development

Desirable criteria

21. Postgraduate qualification in palliative care, neurology, or related field

Experience

Essential criteria

22. Significant experience working with patients with complex neurological conditions
23. Experience in care coordination across multidisciplinary teams
24. Experience in service development and audit
25. Experience working with MND patients
26. Experience in leading MDTs and clinics

Desirable criteria

27. Experience in research or publication

SKILLS, ABILITIES & KNOWLEDGE

Essential criteria

28. Advanced communication skills, including delivering sensitive information
29. Ability to assess, plan, implement and evaluate complex care plans
30. Leadership and Change Management Skills
31. Ability to work autonomously and collaboratively
32. Excellent organisational and documentation skills
33. Understanding of Motor Neuron Disease and NICE guidelines
34. Awareness of local and national health and social care structures
35. Knowledge of MND Association standards

Desirable criteria

36. Understanding of local voluntary sector resources and referral pathways
37. Counselling skills
38. Teaching and presentation skills

SPECIAL CIRCUMSTANCES

Essential criteria

39. Ability to travel to other sites
40. Ability to work in a variety of settings including homes, hospitals, and care homes

41. Car driver with access to vehicle

PPE requirements: Buckinghamshire Healthcare NHS Trust requires all colleagues to wear appropriate personal protective equipment (PPE) in accordance with our infection prevention and control procedures.
