1. Act as an audit lead, effectively utilising the audit cycle and developing the health champion role
2. Monitor and ensure the safe storage, rotation and disposal of medicaments
3. Participate in local initiatives to enhance service delivery and patient care
4. Support and participate in shared learning within the practice
5. Develop an area of specialist interest, taking the lead within the practice
6. Continually review clinical practices, responding to national policies and initiatives where appropriate
7. Participate in the review of significant and near-miss events applying a structured approach i.e., root cause analysis (RCA)
8. Drive the development of nursing services within the practice, liaising with external agencies and professional organisations as required
9. Develop practice administrative and clinical protocols in line with the needs of the patient and current legislation
Main duties of the job
10. Develop, implement and embed health promotion and wellbeing programmes
11. Manage patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advice
12. Implement and evaluate individual specialised treatment plans for chronic disease patients
13. Identify, manage and support patients as risk of developing long-term conditions, preventing adverse effects on the patients’ health
14. Provide advanced, specialist nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF
15. Provide wound care (ulcer/Doppler etc.)
16. Provide specialist clinics such as ear syringing etc.
17. Undertake the collection of pathological specimens
18. Request pathology services as necessary
19. Process and interpret pathology and other test results as required
20. Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required
21. Maintain accurate clinical records in conjunction with extant legislation
22. Ensure read codes are used effectively
23. Maintain chronic disease registers
24. Develop, implement and embed well man clinics
25. Develop, implement and embed an effective call/recall system
26. Prioritise health issues and intervene appropriately
27. Support the team in dealing with clinical emergencies
28. Recognise, assess and refer patients presenting with mental health needs

Detailed job description and main responsibilities
29. Be an extended and supplementary prescriber, adhering to extant guidance
30. Support patients in the use of their prescribed medicines or over the counter medicines (within own scope of practice), reviewing annually as required
31. Contribute to practice targets (QOF etc.), complying with local and regional guidance
32. Liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients etc.)
33. Delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of the practice of the individual
34. Support the clinical team with all safeguarding matters in accordance with local and national policies
35. Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately
36. Deliver opportunistic health promotion where appropriate
