Responsibilities include:
1. Providing robust assessment of patients by telephone or face-to-face who have been referred due to presenting with mental health difficulties.
2. Identifying appropriate care pathways and ensuring the least restrictive option to meet the needs of the patient and ensure their wellbeing.
3. Signposting to appropriate care providers including primary and tertiary care.
4. Raising concerns to senior staff for support and management.
5. Completing documentation and planning care for patients.
6. Presenting cases to Multi-Disciplinary Team for discussion around care pathway.
7. Liaising with colleagues in nursing, occupational therapy, medics, psychology, and admin.
The role will involve, but is not limited to, performing the above responsibilities.
Requirements:
* A minimum of 6 months paid experience as a Community Psychiatric Nurse in a UK hospital environment within the last 3 years.
* Compliance with NMC Professional Registration and keeping up to date with current nursing practices.
* Full UK driving licence and own vehicle.
