What’s the Role?
As a Care Coordinator in our Complex Care Division, you will:
1. Lead with purpose: Manage your own client caseload.
2. Collaborate effectively: Work closely with clients, families, carers, and colleagues to deliver exceptional, person-centred care.
3. Deliver excellence: Ensure shift requirements are met with compassion, safety, and professionalism.
4. Innovate and improve: Drive continuous improvement in service delivery.
5. Support: Participate in an out-of-hours service, providing consistent, reliable support when needed.
This full-time role offers 37.5 hours per week with a starting salary of £25,200, rising to £28,350 after probation. The role includes a shared out-of-hours rota to support clients and care staff.
