Overview

The North Urgent Community Response Team (UCR) provides 7‑day, 12‑hour services in North Norfolk, including admission avoidance, discharge to assess, and end‑of‑life care. We are looking for experienced therapy assistant practitioners or therapy assistants to join this multidisciplinary team.


Key responsibilities

* Contribute to delivery of care pathways for patients with long term conditions, palliative care and rehabilitation needs to achieve quality of life and independence.
* Prevent unnecessary hospital admission, support early discharge from hospital, and work within the UCR team to coordinate care.
* Work with all health care professionals, statutory and non‑statutory agencies to provide a seamless, integrated service.
* Assess, carry out and monitor patients’ assessments and treatments in specified clinical areas, following predetermined protocols.
* Observe patients in their own homes, provide advice on equipment or methods to ease daily activities, and assist with personal care as appropriate.
* Modify and progress interventions using clinical reasoning, notifying a qualified practitioner as needed.
* Monitor patient status and promptly alert a qualified practitioner when unexpected changes occur.
* Plan and prioritise visits to delegated patients, documenting all work in SystmOne on a daily basis.
* Maintain therapeutic relationships with patients and carers, encouraging confidence and engagement in their treatment plans.
* Liaise with the wider MDT, GPs, social services, inpatient teams, allied health professionals and the voluntary sector to ensure care is coordinated.
* Participate in multi‑disciplinary and multi‑agency meetings such as the Gold Standard Framework.
* Demonstrate competence with treatment techniques and equipment, ensuring safe use and maintenance.
* Support indirect patient contact tasks (e.g., telephone handling, appointment scheduling, referrals).
* Use universal precautions and adhere to infection control policies when exposed to bodily fluids.
* Demonstrate IT and standard keyboard skills.


Professional duties

* Attend and contribute to staff and multidisciplinary team meetings.
* Participate in in‑service training and reflective practice.
* Make independent travel arrangements to patients, clinics, base and meetings.
* Provide peer support and training to students, new starters and less experienced staff.
* Recognise limitations of own scope of practice and involve senior practitioners when appropriate.
* Demonstrate awareness of clinical governance, risk management and apply to work situations.
* Contribute to team working, reflect on practice and propose service improvements.


Organisational responsibilities

* Undertake mandatory training and participate in personal development reviews.
* Follow Trust policies, local procedures and ensure absence reporting.
* Contribute to clinical audits, staff surveys, risk assessments and incident forms.
* Take part in clinical supervision as per Trust Policy.


Qualifications & requirements

* Experience as a therapy assistant practitioner or therapy assistant.
* Knowledge of clinical protocols and ability to work independently with clinical support.
* Good communication skills and ability to work with multidisciplinary teams.
* IT skills; experience using SystmOne preferred.
* Willingness to work 7 days a week, including weekends, 8 am‑8 pm on a 365‑day schedule.
* Valid DBS check or willingness to obtain one.
