Abbey Medical Practice is seeking a compassionate and motivated Mental Health Nurse part-time, 10 hours a week, to join our multidisciplinary team, working closely with patients experiencing a range of mental health needs within primary care.
We are a practice with a list size of 8900 patients, located close to Lincoln city centre.
The practice is part of IMP Primary Care Network working along 8 other practices.
We are a GP training practice with a strong commitment to education, professional development and inter-professional learning. Working alongside GP registrars, medical students, and the wider multidisciplinary team, contributing to a supportive learning environment and benefiting from shared skills, collaboration and reflective practice.
Main duties of the job
The postholder will support the assessment, management and ongoing care of patients with mental health conditions in a primary care setting.
Key responsibilities include:
Undertaking mental health assessments and risk assessments
Supporting patients with common mental health conditions, includinganxiety, depression and long-term mental illness
Providing brief interventions, monitoring and follow-up
Signposting and referring to appropriate services within secondarycare and voluntary sector organisations
Working closely with GPs, ANPs, social prescribers and the widermultidisciplinary team
Contributing to care planning and multidisciplinary discussions
Supporting population health initiatives and mental healthservices
Participating in audit, service development and quality improvementactivities

Person Specification
Experience
* Post-registration experience working as a Mental Health Nurse in a clinical setting
* Experience of assessing, managing, and supporting patients with a range of mental health conditions (e.g. anxiety, depression, severe and enduring mental illness)
* Experience carrying out mental health assessments, risk assessments, and care planning
* Experience working autonomously and making clinical decisions within scope of practice
* Ability to work effectively as part of a multidisciplinary team (MDT), liaising with GPs, nurses, and external mental health services
* Experience maintaining accurate, timely, and confidential clinical records
* Experience working with safeguarding concerns and escalating risk appropriately
* Experience working in general practice, primary care, or a Primary Care Network (PCN)
* Experience managing patients within community mental health services
* Experience of supporting patients with long-term mental health conditions in a nonacute setting
* Experience working with care navigation, social prescribing, or voluntary sector services
* Experience of delivering brief interventions, psychoeducation, or self-management support
* Experience of working with diverse patient populations, including those with complex or comorbid needs (e.g. mental health and physical health conditions)
* Experience contributing to service development, audits, or quality improvement activities
Qualifications
* Current registration with the NMC
* Degree or diploma in Mental health nursing or equivalent recognised qualification
* Evidence of ongoing continuing professional development in line with the NMC revalidation requirements
* Knowledge of mental health legislation, including the Mental Health Act and Mental Capacity Act
* Non-medical prescribing qualification or willingness to work towards this
* Training or qualification in psychological therapies, trauma-informed care, or counselling skills
* Assessment and risk management training
* Experience or qualification related to working in primary care, community mental health, or integrated care settings.
