Overview
The physical therapist is responsible for providing physical therapy services to home health patients in their place of residence.
Responsibilities
* Deliver physical therapy services to home health patients in their homes.
* Follow applicable laws, regulations, and professional guidelines while delivering care.
* Collaborate with a multidisciplinary team to plan and implement patient care.
* Document patient progress and outcomes in a timely and accurate manner.
Qualifications
* Education: A degree from a baccalaureate, masters or doctorate program in physical therapy approved by an accredited organization. Licensed to practice as a physical therapist in the State of Pennsylvania.
* Experience: Appropriate experience as a physical therapist. Home Health experience preferred.
* Required skills: Ability to work independently and within a multidisciplinary team. Demonstrates good verbal and written communication and organization skills. Valid driver’s license required.
