We are currently seeking an enthusiastic and innovative Band 6 Specialist Looked After Children and Care Leaver Nurse to join us in delivering high-quality, compassionate care to our Looked After Children. In this rewarding role, you will take the lead in carrying out statutory health assessments and play a pivotal part in identifying, addressing, and advocating for the health needs of children in care.
We are seeking a committed and passionate individual who can demonstrate a genuine interest in the health and wellbeing of Looked After Children, Young People & Care Leavers.
The looked after children's team supports children/young people and their carers to meet any identified physical health, mental health, and emotional wellbeing needs. Looked After Children are a particularly vulnerable group of young people who are often difficult to engage. Although they have many of the same health issues as their peers, the extent of these issues is often greater because of their experiences.
Over recent years, there has been a significant rise in the number of children in care at both national and local levels. Looked After Children and young people have a greater risk of mental health problems, as well as developmental and physical health issues.
* Support the Named Nurse for Looked After Children to fulfill their responsibilities to deliver the LAC service specification and improve the health of children and young people in care.
* Attend and contribute to relevant multi-agency meetings.
* Consult on complex issues related to Looked After Children, Care Leavers, and unaccompanied asylum seekers.
* Coordinate own caseload, which involves writing care plans and liaising as appropriate with other professionals.
* Conduct statutory Looked After Children review health assessments, including developmental and emotional wellbeing, in accordance with national and local guidance and policies.
* Provide health advice, support, and training to professionals from other disciplines, including foster carers, on individual and general health issues to promote the health of Looked After Children and offer health promotion information regarding childhood issues when required.
* Contribute to individual case supervision, engage with safeguarding supervision, and take responsibility for your own supervision needs.
* Contribute to the delivery of training for health staff and inter-agency training.
* Encourage case discussion, reflective practice, and monitoring of significant events at a local level.
