We have an exciting opportunity for dedicated and experienced Support Workers to join our compassionate team at Yorkshire Community Care, supporting our client in the Leeds & Bradford area.

Duties
* Deliver personalised, high‑quality care and support tailored to the client’s individual needs.
* Assist with daily living activities including personal hygiene, mobility and meal preparation.
* Administer medication and monitor health and wellbeing, responding appropriately to any changes.
* Support community access, accompanying the client on outings, ensuring accessibility and safety.
* Provide emotional support, encouragement and companionship.
* Maintain accurate records of all care provided and report any concerns promptly.
* Work collaboratively with family members and healthcare professionals to ensure a holistic approach to care.
What We’re Looking For
* 6 months’ experience in complex care.
* Excellent communication and interpersonal skills.
* Experience in tracheostomy and ventilation.
* Peg feed & Medication administration.
* Energetic, positive, and empathetic personality.
* Awareness of accessibility needs, including wheelchair‑friendly routes, transport and venues.
* Ability to work both independently and as part of a wider care team.
* Shift Pattern: Mon–Sun, 12 hours a day, with at least one alternate weekend.
* Plus an additional sleep shift.
* A calm, resilient and proactive approach to complex situations.
* Someone who can bring warmth, humour and creativity to the role.
* NVQ Level 2 in Health & Social Care (desirable but not essential).
