An exciting opportunity has arisen for a committed, competent and passionate team leader to join our District Nursing Teams within the Community Division for patients across LPB.

The District Nursing Service currently operates a 8am-8pm working model. There is an expectation for the candidates to work within these working hours, including a variety of shifts across 7 days a week to support the needs of the service.
Responsibilities
To operationally manage and lead a District Nursing Team whilst being accountable for the performance, quality, and clinical effectiveness of the team. A key outcome of the role is to support your team to understand and own their quality, recognise, and respond to patient safety concerns, whilst identifying, assessing, and managing risk. The post holder is expected to provide direct patient care when necessary, acting as an expert practitioner and a role model to the whole team whilst actively supporting integration.
As the District Nurse Team Lead you will work closely with the Integrated Community Nursing Senior Leadership Team to develop an effective and highly performing team, driving service improvements that meet patient need and promoting the team as a positive place for staff experience and development.
Patient Care Delivery/Communication
1. Developing, maintaining, and identifying problems with effective locality communication networks with other health professionals, statutory and voluntary agencies.
2. To manage a caseload of patients with a broad range of complex and specialist needs, using evidence-based and client-centred principles to assess, plan, implement, and evaluate interventions.
3. Accurately and timely recording of all care given to the patients and report any changes in the patient’s condition to the general practitioner or other members of the health/social care team.
4. Attending and participating in staff meetings, MDT’s and other meetings as required.
5. Ability to verbally explain complex issues in formal situations such as investigations.
6. Ability to formally present and discuss to individuals and groups ideas and issues pertinent to Community Nursing.
7. Identifying and allocating responsibility for the assessment of particular client groups.
8. Having responsibility for the health assessment of adults within the population as their needs arise.
9. Using the knowledge and skills necessary to assess individuals and groups, identifying the multiple needs of the patient, family/carer providing holistic care taking into consideration cultural differences.
10. Working within the Community Division as a member of the integrated community nursing team, participating in activities, which address the health needs of the general population.
Care/Caseload Management
1. Responsibility for the co-ordination in monitoring the care of patients with long term conditions, disease management and supporting clinical staff ensuring continuity and continuing care.
2. Being responsible for the development of an annual caseload profile to identify the health needs and necessary resources to meet service needs.
3. Responsible for the setting of team objectives in conjunction with the Operational Lead.
4. Ensuring that all Human Resources policies are adhered to including the recruitment and employment of staff, the management of sickness absence.
5. Using own expertise and experience to present recommendations for service development.
6. Responsible for ensuring that all data relating to the patient activity of the team is input onto the information system accurately and on time as required by Trust policy.
7. Having delegated responsibility for budget management operating with constraints identified by management.
8. Allocating work to make best use of the knowledge and skills of team members.
9. Having responsibility for ensuring that appraisals and PDPs are carried out within the team and the information collated.
Personal/Professional Development
1. Participating in the setting of personal objectives with the Operational Lead through the job appraisal and clinical supervision processes.
2. Maintaining and continually updating professional knowledge and skills.
3. Undertaking yearly appraisals with team members, identifying their training, educational and development needs.
