Neighbourhoods Group Coordinator and Administrator
We are seeking a motivated and organised Neighbourhoods Group Coordinator and Administrator to join Wyre Forest Health Partnership on a 6-month fixed‑term basis.
This role sits within our Neighbourhoods Team, which is central to our approach to community‑based care focused on making lives better by supporting people earlier, closer to home, and in ways that truly meet their needs.
Our Neighbourhoods Team works proactively within local communities to understand what matters most to people, particularly those at risk of poorer health outcomes. We identify and address barriers to accessing care and support people to stay well for longer.
By helping people access appropriate services earlier, we aim to prevent health problems from escalating, reduce avoidable hospital admissions, and improve overall health and wellbeing.
Working closely with healthcare providers, the postholder will identify patients who may benefit from group consultations, support care planning, and help patients access appropriate services across health, community and voluntary sectors.
The role will also involve working alongside the Work and Health Coach to support patients on fit notes to remain in or return to work where appropriate.

Main duties of the job
Coordinate and support group consultations for patients with low‑level mental health needs and MSK conditions.
Identify and invite suitable patients and manage bookings, reminders and practical arrangements for group sessions.
Work with GPs and the multidisciplinary team to identify and manage a caseload of patients who may benefit from coordinated support.
Support patients to develop and follow personalised care plans based on what matters most to them.
Provide care navigation and coordination, helping patients access appropriate health, community and voluntary sector services.
Work alongside the Work and Health Coach to support patients who are on fit notes to remain in or return to work where appropriate.
Collect relevant outcome and patient experience data to support service improvement.
Maintain accurate and timely patient records in line with information governance and confidentiality requirements.
Job responsibilities
Group Consultation Coordination
* Coordinate and support the delivery of group consultations for identified patient cohorts
* Identify suitable patients using practice searches and population health data
* Manage invitations, bookings, reminders and attendance records
* Arrange venues, IT set‑up and required materials
* Support the preparation and follow‑up of group sessions
Care Coordination and Patient Support
* Work with GPs and the multidisciplinary team to identify and manage a caseload of patients
* Support patients to develop, review and update personalised care plans
* Provide care navigation and signposting to appropriate services
* Work alongside the Work and Health Coach to support patients on fit notes, including preparation for work‑related discussions and referrals
Data Collection and Service Improvement
* Collect and record outcome measures, attendance data and patient feedback
* Support audits, reporting and service evaluation activities Administrative Responsibilities
o Maintain accurate, timely and confidential patient records in line with information governance requirements
o Manage correspondence, emails and telephone queries
o Support scheduling of meetings, MDT discussions and service activities
o Prepare reports, documents and data extracts
o Use Microsoft Office and clinical systems effectively (training will be provided)
General Responsibilities
o Attend training, supervision and team meetings
o Contribute to quality improvement activities

Person Specification
Experience
o Experience working in a care, support, community or administrative role.
o Good communication and interpersonal skills with the ability to build rapport with patients and colleagues.
o Ability to manage a varied workload and prioritise tasks effectively.
o Good organisational and IT skills.
o Understanding of confidentiality, data protection and information governance.
o Experience working in primary care, community health or social care.
o Experience supporting group activities or programmes.
o Awareness of personalised care, social prescribing and wider determinants of health.
o Knowledge of mental health or MSK support services.
Qualifications
o Educated to Level 3 (A-level, NVQ Level 3 or equivalent experience).
o Further education or qualifications
