We are looking for a Diabetes Support worker to join our team. This is a really exciting post that will be right for someone who has a passion for delivering joined-up, supportive care to children with Diabetes and their families in Community settings.
The successful candidate will work with the specialist Children's Diabetes Team to develop and deliver a well-organised and responsive service to this patient group.
Main duties of the job
The Diabetes Educator will work as part of a multidisciplinary team under the supervision and guidance of the site lead paediatric diabetes specialist nurse, and will provide high-quality, evidence-based care and support of people with diabetes within the paediatric diabetes service aged 0-19.
* Empower children and young people with diabetes and their families/carers to manage their condition effectively by delivering education and health promotion as appropriate.
* Provide specialist education teaching for other professionals across all care sectors, including schools and other educational establishments as appropriate.
* Record all findings in the clinical records and patient notes according to Trust policy.
* Report any concerns, changes and variations in the patient’s psychological, physiological or social conditions.
Person specification
Qualifications
* Successful completion of foundation degree equivalent to diploma level.
* A levels/ Diploma in health or NVQ in health and social care/ Nursery nurse
Experience
* Demonstrable experience of working with children, young people, families and wider support networks.
* Experience of working with CYP in various settings.
Skills
* To be able to communicate effectively with the team, families and children and young people within the caseload.
