Band 3 - Community Support Worker - IMHT Burntwood & Lichfield

About the Team
The Integrated Mental Health Teams work as part of a 'neighbourhood' providing place-based mental health services in partnership with local organisations. Clinicians within the IMHT will offer interventions aligned with NICE guidelines, with team allocation based on service user needs and clinician skills.
Responsibilities
1. Provide planned, recovery-focused care to a defined caseload.
2. Collaborate with other Trust services to ensure seamless care.
3. Partner with external agencies to support service users' access to services in the independent and non-statutory sector.
4. Deliver therapeutic interventions as per the care plan to help service users achieve recovery goals, including improving daily activities, social inclusion, employment access, and health promotion.
5. Conduct physical health assessments (weight, height, blood pressure, ECG, phlebotomy).
6. Contribute to planning and delivering therapeutic activities within the care programme approach.
Note: The role covers a large geographical area, so access to appropriate travel during work hours is required.
