Job overview
The post holder will work within the Urgent Care Service pathway, providing daily operational management and clinical leadership for the effective operational and clinical running of the Integrated Discharge Teams in the 5 London Boroughs of Brent, Harrow, Hillingdon, Kensington & Chelsea, and Westminster. The post holder works across health, adult social care, housing, and immigration services in a multi-agency capacity, working collaboratively to ensure barriers to discharge are identified and eliminated, and clear and safe move-on plans are developed.
Main duties of the job
* Ensure effective clinical oversight, delivering performance management against key performance indicators (KPIs) and financial targets.
* Provide expert advice on social work legislation, trust policies, and adult social care procedures to facilitate safe and effective discharges without delay.
* Work with the Urgent Care Service Senior Management Team to deliver the Trust's vision for a quality-led service and collaborate with the Deputy Heads of Urgent Care to ensure effective patient flow and bed utilization across the Trust.
Support the development of the strategic direction for the Urgent Care Service in line with national policies and local health needs.
Operationally manage and lead the IDT team, making appropriate operational decisions to ensure effective running of the service.
