Job Overview:
We are seeking a dedicated and compassionate Community Nurse to join our healthcare team. In this role, you will deliver high-quality nursing care to patients within the community, supporting their health, independence, and overall well-being. The ideal candidate will demonstrate excellent clinical knowledge, strong communication skills, and a commitment to person-centred care.
Responsibilities:
* Conduct comprehensive assessments and develop individualized care plans to meet patients' clinical needs
* Administer medications, injections, and treatments in line with clinical guidelines and prescriptions
* Monitor, evaluate, and record patients' health conditions, escalating concerns appropriately
* Provide wound care, catheter care, palliative support, and other community nursing interventions
* Educate and support patients and their families in managing health conditions and treatment plans
* Collaborate with GPs, allied health professionals, and social care teams to ensure integrated care delivery
* Maintain accurate, confidential, and compliant patient records in line with regulatory and professional standards
* Promote independence, dignity, and quality of life for patients in their own homes or community settings
Qualifications:
* Registered Nurse (NMC Pin required)
* Previous experience in community or primary care nursing is desirable
* Strong clinical assessment and decision-making skills
* Excellent communication and interpersonal abilities
* Ability to work independently as well as part of a multidisciplinary team
* Full UK driving licence (preferred for community-based travel)
Nice-to-Have Skills:
* Experience in chronic disease management, wound care, or palliative care
* Familiarity with electronic patient record systems
* Confidence working in diverse cultural and multilingual communities
